Certified copies of Birth or Death Certificates can be be obtained by visiting our office,or can be ordered by either mail or internet/fax.
 

Fees for Records:

  1. Geneaology Search                                     $5.00   (per name)
  2. Certified Birth Certificates                           $10.00 (per certificate)
  3. Certified Death Certificate                           $10.00 (per certificate)
  4. Un-Certified Birth/Death Certificate             $10.00 (per certificate)
  5. Paternity Affidavit                                        $25.00 (each)
  6. WE ACCEPT CASH, CHECK OR MONEY ORDER, and ONLY DEBIT CARDS ($2.50 extra fee) AT THIS TIME

Birth/Death Certificates

Reasons that a Certified (raised seal) Birth/Death Record could be required:

  • International Travel (passport application)
  • Insurance Purposes
  • School Registration
  • Sport Registration
  • Sale of Vehicle, Home, Property, etc.
  • Retirement
  • SSI 

Guidelines for Requesting Certified (raised seal) Birth or Death Records (We only have records for people who were born or died within Huntington County!):

  • Completed Application (available online)
  • Self-Addressed Stamped Envelope
  • Birth/Death recording start date is 1882
  • Photo ID required
